Plaque parapsoriasis (or Broca’s disease) is a skin disease that most often occurs in men over the age of 30. The English scientist Brock was the first to write about the disease in 1897. In our country, this disease is described in detail in the works of Professor I.M. Raznatovsky.  

Plaque parapsoriasis is characterized by a characteristic rash that appears on the skin around the abdomen, thighs, or buttocks. Looks like patches of yellow-pink or brownish-yellow hue. In moments of exacerbation of the disease, spots can acquire a reddish-brown tint. It happens that the skin looks speckled due to the fact that the areas of the skin around the hair follicles have a slightly different color. On the distal (farthest from the base) parts of the limbs sometimes acquire a bluish tint.  

The shape of the spots can be round, oval, elongated-elliptical, they can be arranged in stripes, the outlines of the spots are uneven. The most common spots are stripes with pointed tips. They are located on the limbs symmetrically to each other, the long axis of the spot runs parallel to the axis of the limb. If the spots are located on the abdomen, then they run parallel to the ribs. General resorption of rashes is characteristic of plaque parapsoriasis .   

 On the surface of the spots, sometimes fine peeling can be noticed. It is usually only found after the skin has been scraped. The keratinous layer of the skin is often folded, resembling wrinkled paper in appearance. The skin pattern is usually underlined. 

The size of the spots is most often related to their shape. Round spots usually do not grow more than 3 cm in diameter, elongated-elliptical – they reach 5 cm in length and 3 in width, spots in the form of stripes can grow up to 8 cm in length , in width – also up to 3 cm.In some cases spots can be up to 15 cm in length. In some places, spots can merge with each other, but this is due to friction of the skin on clothing or due to improper external therapy. 

Plaque parapsoriasis most often occurs on the inner side of the shoulder (while going out to the back), on the sides of the chest, in front of the abdomen, in the lumbar region. On the legs, spots may appear on the inside of the thighs and lower legs. The rash is usually symmetrical. The skin on the face, head, feet, and palms is usually unaffected.

Having formed and reaching a certain size, the spots can calmly exist for months and years. At the same time, in winter, the disease is somewhat aggravated, and in summer, the skin can partially recover and be masked by a tan. Sometimes there are spontaneous improvements, rarely – complete restoration of the skin. In place of the disappeared spots, the skin remains normal, atrophy does not occur. 

Doctors cannot yet say with certainty what causes the development of Broca’s disease. Often it is associated with the presence in patients of any chronic diseases of the gastrointestinal tract (such as colitis, gastritis, ulcers, which are accompanied by constipation). This gives doctors reason to assume a connection between the occurrence of parapsoriasis and chronic intoxication.

As a treatment, remedies are used that correct the violation of the work of internal organs. They conduct courses of injections of B vitamins (once every 2-3 months), and also prescribe the intake of ascorbic and niacin. In moments of exacerbation of the disease, doctors prescribe the use of local ointments based on glucocorticoids. During the period of quiet development of the disease, a good effect can be achieved by applying UV therapy methods in small erythemal doses.

With this form of the disease, there is a significant thickening of the upper layers of the epidermis. Severe coarsening of the skin (hyperkeratosis) causes the formation of characteristic callus-like seals of a reddish hue, which are sometimes covered with a mesh of small painful cracks. According to these signs, plantar psoriasis is in many ways similar to eczema or mycosis, so for an accurate diagnosis, the patient must undergo a serological examination and make a skin biopsy.  

The lesions of psoriasis on the soles are isolated and look like oval or rounded plaques with a shiny scaly surface. On the border, the lesions are surrounded by a ring of denser skin. In the absence of adequate treatment, plaques can grow and join together. Psoriasis plaques clearly stand out against the background of healthy skin with a characteristic pink tint. In some cases, yellow pustules (palmar-plantar pustules) are present in the lesions – formations of a non-infectious nature that do not need additional treatment and soon disappear on their own.

The main symptoms also include excessive dryness, redness and thickening of the skin with many cracks and severe itching. If the acute course of the disease is aggravated by the ingress of a fungus or infection into the skin cracks, an inflammatory process begins in the affected areas, accompanied by pronounced pain sensations with any movement.

Treatment methods

First of all, local therapy is used to treat palmar-plantar psoriasis. That is, glucocorticoid ointments help to relieve the symptoms of the disease, which make it possible to achieve a quick and significant reduction in the external symptoms of psoriasis, pharmacy talkers, skin softening creams, compresses from infusions of medicinal herbs. For plantar psoriasis, a hydrocolloid coating is used, which promotes the speedy healing of sores and cracks in the skin, helps to reduce itching and significantly reduces the friction of the foot on socks and shoes.

Ointments

The multifactorial nature of psoriasis explains the high prevalence of this skin disease. According to statistics, 40% of all inhabitants of the planet are affected by the disease. Scientists still have not been able to establish all the causes of psoriasis, just as there are no ways and methods of cure for this inflammatory skin disease in the arsenal of modern official medicine.

Another characteristic symptom of psoriasis is the so-called “terminal film”. If you try to remove scales from the surface of the plaque, then at the same time a thin layer of the epidermis begins to peel off. Under it, a smooth, as if polished, pink-red skin is found, on the body of which the smallest punctate bleeding begins to appear quickly.

In some cases, the initial stage of psoriasis is characterized by pronounced exudative manifestations, that is, papule scales that have absorbed exudate (serous fluid) turn into a crust. If you try to rip off such a crust, then skin wetness will be found under it. Most often, such symptoms appear on the patient’s legs, and the type of disease in this case is called exudative psoriasis.  

At the stage of disease progression, psoriatic plaques can grow to very large sizes, sometimes spreading to almost all skin integuments of the human body. In this case, the appearance of a bright red rim around the plaque is observed, and then new rashes appear near the existing foci.

Treatment

Psoriasis (lichen scaly) is a skin disease, a chronic disease that lasts for years, is characterized by an alternation of deterioration and improvement in the condition. This is a systemic disease of the body with the influence of a variety of factors, therefore, the causes of the onset of the disease in each case are given maximum attention, because the correctness of the chosen treatment tactics will depend on this. Why is the history of psoriasis endless? Because, in fact, the disease is incurable. It is possible to achieve a long-awaited and long-term remission, but this is not a treatment, but a period when the patient should pay increased attention to his health and try to avoid the putative risk factors.  

Psoriasis is characterized by the presence of a rash in the form of nodules of various sizes, called papules. Papules are pink, light red, and covered with a kind of fluffy silvery-white scales that make the lesions appear whitish. There is no the same onset of the course of the disease for everyone. More often, there is a variant with a slow increase in the number of rashes. The first rashes can be overlooked, especially if they are located on the scalp. And only under the influence of provoking factors, the number of rashes increases.

It can be trauma, food excesses, pregnancy, etc. In other cases, the history of psoriasis begins with a sharp “sprinkling” of the epidermis with many elements of various forms. More often this happens after infectious diseases, nervous experiences, aggressive drug therapy. In the course of the disease, psoriatic spots can change their localization sites, heal completely and renew again. The rash affects any part of the body, including the hairy areas, but is more common on the extensor surfaces of the limbs (elbows, knees).  

Types of psoriasis

Distinguish between progressive, stationary (stable) and regressive stages. By the symptoms of manifestations, the following types of psoriasis can be distinguished:

· Usual (general clinic);

· Arthropathic;

• exudative;

Pustular;

· Psoriatic erythroderma.

So, exudative psoriasis is characterized by the formation of lamellar scales, yellowish crusts. If you remove them, you will find a weeping bloody wound.

Pustular generalized psoriasis occurs on the palms and soles of the feet. It is characterized by fever, malaise, and soreness in the area of ​​plaques. Sometimes it is accompanied by damage to the joints, most often small (hands and feet), there are also large ones. The distal joints are more often affected. At first, these are isolated arthritis, usually asymmetric, on one of the arms, for example. In the future, dislocations, subluxations may appear, followed by deformation of the joints.

Psoriatic erythroderma is an acutely developing, general process. The patient’s temperature rises, there are pains in the joints, the general state of health becomes worse. The occurrence of erythroderma is likely due to the growth and fusion of plaques, as well as under the influence of provocative factors such as excess sun. In psoriasis, nail damage is sometimes observed, pinpoint depressions similar to a thimble, opacities or longitudinal and transverse grooves are visible. Also, the nail plates can be thickened, deformed, and have an uneven surface. In other cases, the nails are generally rejected.

Forecast

Over the past century of the development of medical technologies, more than one theory of the onset of psoriasis has been derived. These are metabolic, immune, endocrine, neurogenic and genetic, intoxication, and other theories. Taken into account in scientific circles, each case history of psoriasis in each patient requires an individual approach and detailed study in order to prescribe those drugs that will bring the maximum effect and will not harm the general state of the body.

Viral or infectious diseases

Disorders or pathological changes in the digestive system

Weakened immunity

Stress or a state of constant nervous tension

Disorders in the endocrine system

Skin trauma – cuts, burns, stitches after head surgery

Effects of low temperatures on the scalp

Allergic reaction to chemical and physical irritants (cosmetics for hair care, unfavorable ecology, psycho-emotional stress, etc.)

Tobacco smoking and alcohol

The essence of the problem lies in the fact that due to the disruption of the normal course of processes in certain organs and systems of the human body, the cells of the epidermis begin to divide much faster than necessary. As a result, scaly layers, seals, irritations form on the surface of the skin, and then inflammation begins in certain areas, accompanied by severe itching, redness, and peeling of the skin.

Symptoms of the disease

At the initial stage of development, psoriasis of the scalp is characterized by mild symptoms. Exfoliation of the skin gradually becomes more noticeable, which is very similar to severe dandruff. Soon, mild irritation, itching, nodular seals appear in some small areas. Then, inflammation of the skin joins these symptoms, the itching becomes much stronger, and the skin becomes covered with cracks, erosions, abrasions when scratching.  

Further, either under the hair, or on the earlobes, behind and above the auricles, on the back of the head or forehead, the main signs of psoriasis appear – large red plaques with a silvery-gray scaly surface. At the slightest mechanical effect, the skin in the lesions is easily damaged, burning and painful sensations appear.

If you do not deal with the treatment of the disease at this stage, the psoriasis of the head begins to progress, the plaques grow, join together, forming large areas of skin lesions, up to the formation of the so-called “helmet” covering the entire epidermis under the hair.

Treatment of psoriasis, in any of its manifestations, should be carried out only after the exact identification of the cause of the disease and under the constant supervision of a dermatologist. Self-treatment of scalp psoriasis is not allowed.
